I understand that my information will be used in accordance with Fresh Rise Group Terms and conditions. This includes emails containing exclusive offers and invites to future webinars. Monthly coaching charges will occur once per month on the date this form is first completed. Upon payment, client is paying in advance for 2 coaching sessions per month. Client can cancel monthly coaching at any time prior to the 1st coaching session of the month. Unused paid sessions can be used up to 6 months after termination of regular monthly coaching.
